I probably should have made this GUIDE a long time a go, but better later than never :p
Here's a guide on how to use this Move Hack Converter.
Move hacks can use different types of codes (they can modify the animation only or also the distance mods for example).
You can recognize what a line does by looking at these two numbers:

45-46: P1's animation
51-52: P2's animation
5D-5E: P3's animation

If you look at the proximity mods, they work in a similar way: the numbers vary a bit, but these two are changed with player conversions.

The Tokis don't need to be converted, you only need to use the right activators to keep them always on. Read this post for further information:

Use the Move Hack sheet and insert the values from the move hack like this:
If you're doing it correctly, you'll notice how in the P1>P2 column the code is the same you had in the original move hack.
In this case you don't need P2's animation as this is not a grapple hack.
If you want the move to be player specific, insert a wrestler ID and the code will appear on the right. In this case because we're editing an attack (you don't have a player 2 animation), not all the specific columns are necessary (you can see how some of them look the same).
For a specific hack you need to worry about making the tokis player specific. Check the previous link for more information.
So, you now copy and paste the columns you need and you have your converted code:

This was a short code, but if you're converting longer moves the method is the same. Insert the P1 values and you'll be fine. For distance mod make sure you're using the right sheet and insert the P1 values there.

Let's try a damage mod now. I want to make the "Chokeslam from Hell" S-Powered whenever is used as Front Special.
Go to the damage mod sheet. The Front Special damage mod for P1 is at 810ABF98. Type this in the "D8" cell.
You'll see how the conversion appear on the right. Now we need to add activators. Click on the "Grapple mod" link on top. The Front Special value is "D109102c". Insert it in "N1". Now look for the Master Move Mod for the Chokeslam. It's "01C2". Insert it in "N2". Copy and paste the values:

The player specific code works ok in general, but there is a problem if you play a battle royal (after you're eliminated the following player still has this code active). To avoid this add this activator:
